Care coordination involves deliberately organizing patient care activities and sharing information among all the participants concerned with a patient's care to achieve safer and more effective care. This means that the patient's needs and preferences are known ahead of time and communicated at the right time to the right people, and that this information is used to provide safe, appropriate, and effective care to the patient.

There is no cost to enroll in Care Coordination.

Matt Strong, NRP - Care Coordination Manager

Working in healthcare since 1999, initially in Patient Financial Services to include becoming certified as Certified Patient Account Manager.  He attended the Paramedic program at Fayetteville Technical Community College in Fayetteville, NC.  He has been a Paramedic for 15 years.  He’s certified in Basic Life Support, Advanced Cardiac Life Support, Pediatric Advanced Life Support, Pre-Hospital Trauma Life Support.  Matt worked as a travel Paramedic during the COVID Pandemic assisting with contracts in Texas, Iowa, North Dakota, California and Florida.  He answered a job ad for Pinedale and found his new home.
